Faith in Families Program PDF Print E-mail

The Faith in Families Program provides intensive case management services to Emergency Assistance, TANF and Post TANF clients. Additionally, this program assists with case management needs for clients participating in pre-apprenticeship programs.

TANF Program

Of the individuals serviced by our Agency in Camden County, approximately 70% are Temporary Assistance for Needy Families (TANF), GA or SSI recipients. Accordingly, Catholic Charities employees are intimately familiar with the requirements and expectations of these various government entitlement programs and understands this program population and their particular needs, as well. Routinely, we service between 250 and 350 clients per month with outreach and referrals for support services.

Partnering with St Joseph Pro-Cathedral and St. Anthony of Padua parishes in Camden along with our relationships particularly with a number of St. Vincent de Paul conferences throughout Camden County, Catholic Charities performs outreach and referral services for Post-TANF clients across all of Camden County. While a large percentage of the Post-TANF client reside in the City of Camden, we believe it equally important to ensure outreach to non-Camden residents who live in other Camden County communities.

Services for the Post-TANF population are needed in Camden County for two reasons – one dealing with volume, the other with education. As noted above, Camden County has a staggering number of persons who fall into the Post-TANF category. Therefore, special attention needs to be given to this client population to ensure they continue to move forward towards self-sufficiency.

Regarding the education of Post-TANF clients, unless specific individual attention is given to this client population, entitlement services to which they are eligible often are not received due to lack of information or existing barriers. Catholic Charities breaks these barriers down, connects these Post-TANF clients to the Board of Social Services, which will link them to additional social services for which they are qualified.

SAIF Program

The mission of our agency directly relates to our Supportive Assistance for Individuals and Families (SAIF) Program as we are “to identify and alleviate human suffering” as we “provide counseling and social services on a non-sectarian, non-discriminatory basis with special emphasis on providing services to the poor and oppressed.” Our agency’s target clients absolutely include the post 60-month clients of this program. Accordingly, intensive case management services are one of the core services we currently provide.

Catholic Charities has significant experience in providing intensive case management and supportive services to welfare recipients. We currently are the provider of services for the SAIF Program in Burlington, Camden and Gloucester counties.  The goal is to move these clients towards self-sufficiency and off of public support programs. We successfully obtained work activities for approximately 125 clients each month. This annualizes to an estimated 1,500 persons who successfully achieved constructive work activity, employment or support resources to move clients into work.

Diabetes Screening Program

Catholic Charities, Diocese of Camden, Inc. in partnership with CAMcare Health Corporation, federal qualified health center, offers a free Diabetes Screening Program.  This initiative is made possible by a grant from the NJ Department of Health and Senior Services, Office of Minority and Multicultural Health.

Catholic Charities outreaches to the medically underserved, uninsured, and underinsured, living within the city of Camden.  Using the Diabetes Risk Test, identified individuals who are at high risk of diabetes and those who have been previously diagnosed to have diabetes, but are not receiving care, are clinically linked to medical providers and benefit from case management services and diabetes education.

The mission of this program is to identify, educate, case manage, clinically link and track, including follow up, individuals at risk for developing diabetes and perform culturally competent community education/awareness on diabetes prevention, health promotion behaviors and the diabetes disparities that exists in NJ.  Since the programs’ inception in 2005, 164 patients have been treated for diabetes. Currently, 81 at-risk and diabetic patients have been identified.
